How do I submit an order for service.

Orders can be submitted online via our website, fax or email

FOR INDIVIDUALS

1. Fax/email the doctor’s orders( requisition form) and insurance card (both sides),

2. You will then go to our website to our secure online scheduling system. Once appointment is booked you will receive a call for confirmation and instruction, or your scheduled time will NOT BE VALID) Call us if have any additional information.

Prepare for your blood test as advised by your doctor. FOR FASTING BLOOD you would need to schedule an early appointment,

For patients receiving an Electrocardiogram the day of appointment please DO NOT put lotion, oils, or perfume on the chest area where leads will be placed.

FOR DOT/NON-DOT URINE SCREENINGS:

Employers must send request on reason donor is being tested and the type of testing required.

Donors must email or fax copy of driver’s license.

FMSCA agencies CDL information is required as well as the state license was issued. Orders must also include the fax number or email to where results should be delivered.

FOR PHYSICIANS /PRACTICES

All orders must have patient’s name, date of birth, gender, address, phone number and insurance information as well physician’s name, address, phone number, physicians, NPI number, fax number and signature. Please make sure to include the name and the test code for each test being requested.

*Any order missing any of this information will be delayed and may not be processed.

2. Once appointment is booked online with our secure online scheduling system. Patients are contacted the day before the draw to confirm the appointment and provide a window in which to expect the technician

3. Confirmation of the appointment date and time is faxed/email back to the doctor’s office once the set-up is complete.

4. Completed draws are delivered to the designated lab.

5. Test results are then sent directly to the doctor’s office from THE LAB.

How much notice does your service require?

Routinely, we prefer orders by 3:00 pm the day before collection, but for stat or high priority draws can be often added to daily schedule. Please call us for details.

Do you pick up urine samples?

Yes. please call to check availability to check same day specimen pickups.

When will orders be scheduled?

Sending your orders as far in advance as possible will ensure your draws/collections are scheduled for the date they are needed. Normally, requisitions received by 3pm

How early do you come for fasting draws?

As early as you need them, usually before breakfast.

Do you provide same-day or STAT services?

Yes. AMMS does provide an emergency same-day or STAT service. However, there is an additional service fee of $65 for stat or same day orders. Please fax/email and call to alert us of urgent blood draws.

Do you provide weekend service?

Yes. We do provide weekend service. Please call us with the details.

Do you provide after 3:00 pm services?

Due to collection requirements for many laboratory kits and tests, it is highly recommended that blood collection services be performed prior to 3 p.m. However, we DO provide service after 3:00pm

Can I get collections on Monday if I missed the 3:00 pm cut off the Friday before?

In many cases, complete and accurate orders can be added to the Monday collection schedule. In the event Services is not able to add weekend orders into the Monday collection schedule, Align Mobile Medical Services LLC will schedule those orders for Tuesday collection.

Do AMMS accept insurance?

NO. Align Mobile Medical Services does not accept insurance for any of our services. However, insurance information will be collected at the time of service to submit with the specimen for lab testing. Specimen collection services are provided and billed by Align Mobile Medical Services. Laboratory testing is billed through your insurance.

When will I receive my lab results?

Results are sent directly to ordering physician. Processing time is usually 2 to 3 business days, Some test can take up to a week . In the event that your physician did not receive results, it is the responsibility of the doctor office to get in contact with the lab.
